如欲索取牌照紀錄，須向本會遞交已簽署的申請文件正本，當中須註明以下資料：
請將申請文件郵寄至：香港鰂魚涌華蘭路18號港島東中心54樓，證券及期貨事務監察委員會中介機構部發牌科。我們會通知閣下親身領取牌照紀錄。請注意，如索取文件副本，或須繳付費用。
一般而言，牌照（除設有期限的臨時牌照外）只會被暫時吊銷或撤銷才會失效。牌照持有人必須向證監會繳付年費及遞交周年申報表，否則其牌照會被暫時吊銷或撤銷。
